Appointments

Appointments can be made by calling our office during normal business hours.

After Hours & Emergency Care

If you are experiencing an emergency, dial 911 or go to the nearest emergency room. For all other calls, an on-call physician is available 24-hours a day, 7-days a week.

Prescription Renewals

Prescriptions are renewed during normal office hours. We send your prescriptions electronically to your preferred pharmacy, so please ensure you keep us informed of your pharmacy preferences. In some instances, we may require that the patient be seen in our office prior to medication renewal.

Financial

All office visits co-pays are due at the time of service. We accept cash, checks, MasterCard, and Visa cards. Please contact our billing office should you have any financial questions or concerns or preview the Patient Forms posted on this website for more information.

No Show Policy

As of January 1st, 2021 if you do not call to cancel or reschedule your appointment prior to your appointment time you are subject to a $20.00 no show fee. If you do not call to cancel or reschedule your in office procedure you are subject to a $150.00 no show fee. Also if you have three (3) “no shows” in a 12 month period you will be dismissed from the practice. The No Show Policy Consent will be provided to you at your annual visit each year and can be located under the ‘Patient Documents’ tab –> ‘Other Forms.’

Late Policy

If you are more than 15 minutes late to an appointment, you will be asked to reschedule your appointment.

Form Completion Fee

If you need a form completed by a RN/NP/MD, there will be a $20.00 fee for every form you need completed. Please allow 7-10 days for your forms to be completed and signed. This form can be found under the ‘Patient Documents’ tab –> ‘Other Forms.’

COVID Policy

Per the CDC, we are NO longer requiring masks in our office as of September 29th, 2022. We ask you to wear a mask if you are showing symptoms of COVID-19 or have had a close contact w/COVID-19. If you have a confirmed case of COVID-19 we ask you to cancel your appointment and follow the most up to date CDC guidelines.  We thank you for your consideration.
